Billing and Insurance
Please arrive 10-15 minutes before your scheduled appointment time. Please have your insurance card ready for us to scan.
Most insurance plans, including Medicare, are accepted. To verify if we allow your plan, please get in touch with us. If we do not conduct business with your insurance company, you must pay in full at each visit. It is your duty to understand your health insurance benefits. Ask you insurance representative if you have any questions about your coverage or benefits.
Proof of Insurance
So that your account is up to date, your insurance card will be scanned and saved once a year. Please let us know before your next appointment to make the necessary adjustments if your insurance changes.
Cancellations or No-Shows
Kindly give us a 24-hour notice if you need to cancel or reschedule so we have enough time to book another patient for that time window. Regular cancellations or no-shows will result in dismissal from the practice.
No Insurance
Payment is expected at the time services are rendered. We will provide you a 20% discount if you pay at the time of service. If you need a quote, please call 48 hours before your appointment to request one.
Co-Payments
Co-payments must be made at the time of service.
