At your first visit                                       

Please arrive 10-15 minutes before your scheduled appointment time. Bring in your completed new patient information packet that was mailed to you along with your insurance cards(s)


Proof of Insurance

Your insurance card(s) will be scanned and saved on a yearly basis so your account up to date. If your insurance changes, please notify us before your next visit so we can make the appropriate changes. 


All Co-Payments MUST be paid at time of service.


We participate in most insurance plans, including Medicare. Please call our office to see if we accept your plan. If you are insured with a plan that we do not do business with, payment in full will be due at each visit. Knowing your insurance benefits is your responsiblity. Please contact your insurance representatvie with questions about your coverage and benefits.

No Insurance

Payment is due at the time services are rendered. If you are not covered by group health insurance including The Marketplace, Medicare or Medicaid. We will provide 20% discount.

Cancellations - No Show's

Kindly provide us 24 hours notice to cancel and or reschedule. This allows us time to schedule another patient in that time slot.

Repeated cancellations and/or no show's could result in being discharged from the practice.
